Implementation: R2_GITHUB_OBSERVER_V1_3_BROKER_IMPLEMENTATION_PINNED.

The observer now requires broker implementation R2_GITHUB_APP_CUSTODY_BROKER_V1_1_EXACT_SCOPE both in broker health and in every JIT token response. The broker implementation id is also included inside the signed observer receipt, so downstream authority can reject receipts produced through an older broker implementation.

Direct GitHub token custody remains retired. JIT tokens are exact-repository, exact-permission, ephemeral only.
